ENROLLMENT INFORMATION
REGISTRATION INFORMATION
Kindergarten Enrollment Eligibility: All children residing in the City of Chicago who are 5 years old on or before September 1 of the current school year, are eligible for enrollment into a CPS Kindergarten program.
The parent, legal guardian, or temporary custodian of any elementary age child enrolling in a Chicago public school is required to submit proof of age and required medical records to the school.
Proof of age: Includes, but is not limited to, any ONE of the documents listed below:
Child’s birth certificate
Child’s baptismal record
Passport
Court documents
Medical records
Proof of current address
Includes, but is not limited to, any TWO of the documents listed below:
Current utility bill
Illinois driver’s license or State of Illinois identification card
Deed
Employee identification number
MediPlan/Medicaid card
Court documents
Illinois Department of Public Aid card
Stamped United States Post Office change of address form
Illinois state aid check/social security check
Health requirements
Children are encouraged to submit their required immunizations and physicals prior to the start of the school year and no later than October 15 of the current school or they will face expulsion from school.
Physical examination requirements
All students must have a physical examination within one year of:
entering schools in the State of Illinois for the first time, at any grade level
entering kindergarten or 1st grade, 6th grade, and 9th grade
(ages 5, 10, 15 for ungraded programs)
entering preschool, up to age 6 (physical exam and lead screenings)
Physical Examination Requirements
To be received upon entry to school:
Entering preschool and kindergarten up to age 6 (physical exam and lead screen)
Entering the State of Illinois for the first time at any grade level
Entering kindergarten, 6th grade, or 9th grade (ages 5, 11, and 15 for un-graded programs)
Entering preschool and kindergarten up to age 6 (physical exam and lead screen
Immunization Requirements
Diphtheria, Pertussis (Whooping Cough), Tetanus (DTaP/Tdap)
4 or more doses
The last dose qualifying as a booster and received on or after the 4th birthday
If 10 years have elapsed since the last booster, a tetanus containing a vaccine booster is required
Inactivated Polio Virus (Polio Vaccine)
3 or more doses
The last dose qualifying as a booster and received on or after the 4th birthday
Measles, Mumps, Rubella (MMR)
1 dose received at 12 months or later
2nd dose must be administered at least 1 month after the 1st dose
Hepatitis B (3 doses required)
1st dose received at birth or later
2nd dose received at least 1 month after 1st dose
3rd dose received at least 4 months after 1st dose
Varicella (Chicken Pox)
Required for students entering preschool and kindergarten
Haemophilus Influenzae, Type B (HIB)
Students entering any preschool or pre-kindergarten program (not required after age 5)
Only 1 dose required if first dose administered at or after 15 months of age

Kindergarten Registration for 2012 - 2013
We will have formal registration on March 27th, 28th and 29th in room 108 from 9:30 a.m. until 1:00 p.m.
If your child will be 5 before September 1, 2012, he/she can be registered for Kindergarten. To register parents must come with 2 proofs of address (gas, light, water bill etc.); Photo ID with current address; Original Birth Certificate. When registering the bills and photo ID should be of the person who is filing out the forms. At registration more information will be given about a Kindergarten Orientation and placement tests. Forms for Medical, Dental and Eye Exam will be given at registration and should be returned prior the start of school.
Kindergaren is full day with a 20 minute lunch .
We currently have two locations 1 class will be at our Canty Branch at 8300 West Addison and the others will be housed within our main building.
We are currently working on our longer school day schedule but Kindergarten will have the same time schedule as our 1st - 8th graders.
